Attendance Boundaries

REQUIREMENTS FOR REGISTRATION

Kindergarten students must be five years of age on or before September 30, 2007. At the registration, parents of all new students must present the original copy of their child's birth certificate, which will be photocopied and returned. They must also provide three proofs of residency (ONE of the following: original lease or deed signed by both parties, mortgage payment booklet, or agreement of sale followed by original copy of settlement papers within 45 calendar days of settlement; PLUS any TWO of the following showing identical name and address: valid driver's license, valid vehicle owner's card, current utility bill, paycheck stub, tax bill or sewer bill.)

Pennsylvania state law mandates that all children entering school must be immunized. Therefore, parents must bring proof of immunization with them to registration. Children must have the following immunizations:

3 doses of Hepatitis B Vaccine required for all students K through 11
4 doses of DTP or DT Vaccine (D = Diphtheria, T= Tetanus, P = Pertussis); the 4th dose must be administered on or after the 4th birthday.
3 doses of the Oral Polio Vaccine or 4 doses of the Salk Polio Vaccine
2 doses of Measles, Mumps, and Rubella Vaccine (MMR); the first dose administered at 12 months or older, the second dose administered one month from 1st dose as MMRII.
1 dose of Varicella (chicken pox) vaccine, grades K through 3 and 7 through 10
1 test for tuberculosis done within the last six months is required for all students entering K.

The registration of students will not be complete until the school receives all of the required immunization records. In addition, a physical examination, including a tuberculin test, must be completed no earlier than six months prior to the child starting school.

Whenever possible, please register students in the spring for the following school year. Program planning, budgeting and staffing is based on enrollment projections, and more accurate numbers ensure a smooth start in the fall.
