VBA Voluntary Vision Plan
This low cost ,voluntary benefit is available to all active regular staff, both part-time and full-time. It is a two-year commitment and can only be elected during Open Enrollment for the plan term beginning July 1st, regardless of your hire date or return from leave date. The plan encompasses vision exams, glasses, contacts, discounted Lasik, and discounted Beltone hearing aids.
Once elected, you can make changes to your coverage (add/delete dependents) during the open enrollment period associated with two-year anniversary of your enrollment, unless you experience a qualifying life event
. At the end of the two-year term, coverage will automatically renew unless you opt out of the plan.
VBA does not provide ID cards. To find a participating provider in your area, visit http://www.visionbenefits.com
. When you schedule your appointment, notify the provider that you have VBA and confirm that they accept in the plan. The participating provider will submit the claim and be paid by VBA for covered services. You will be financially responsible to pay the provider any costs above the plan limitations. If you prefer to use an out-of-network provider, you will need to submit a claim directly to VBA for reimbursement.
