Radnor High School iPad Initiative Information and Forms

    All required forms can be found online through the links below. Parents/guardians of all students need to complete the online form. A confirmation email will be sent to the parent or guardian. A printed copy of the email must be signed by a parent/guardian and student.

    Sophomores, Juniors, and Seniors 

      • PLEASE NOTE: In order for the insurance for your school-issued iPad to be up-to-date, all 10th, 11th, and 12th grade students must complete the current insurance form. Students should submit the payment of $37 unless they are certifying on the form that they are covering the iPad under their personal homeowners insurance or self-insurance, taking financial responsibility for the device. The current form can be found by clicking here. More info can be found by clicking here

        Students should submit insurance and payment forms to their homeroom teachers. Students who turned in their iPads at the end of school last year may pick them up from the Technology Office.
